Step 1

MyAgedCare is a government service that determines your eligibility for an aged care service.

MyAgedCare makes a request to us, Aged Care Assessment Services (ACAS), to undertake your assessment.

You are here on your
assessment journey

During the assessment, we work with you to develop your support plan and offer choices of care that are right for you.

Step 3

ACAS will make a referral to either your chosen provider, or to a service provider that is most qualified to provide the type of care you require in your local area. The service provider is the organisation that will deliver the services to you.

If your needs change, you may contact MyAgedCare and request a reassessment at a later date.
